Construct your own Questionnaire posed to KellyNYC

You have convinced your supervisor that Ms. Kelly’s questionnaire is so bad, that a new one is needed. So of course, she has given you the task of drafting a new questionnaire.

The purpose of the questionnaire is the same - to get a general picture of the number and type of incidents at the school. You may choose a time period that makes sense to you. However, it is important to try to identify duplicate data, so construct your questionnaire carefully. Your questionnaire should include at a minimum:

A cover letter explaining the purpose of the questionnaire and assuring the participants that answers will be kept in strictest confidence

A survey that includes the following:

  • A minimum of 4 closed questions
  • A minimum of 2 open questions

In a brief separate essay, discuss:

  • How would you pilot your questionnaire to find out any problems, so they can be corrected before sending it out to everyone? Explain your response.
  • How would you distribute the questionnaire? Explain your response.
